Echocardiographic Parameters for Risk Prediction in Borderline Right Ventricle: Review with Special Emphasis on Pulmonary Atresia with Intact Ventricular Septum and Critical Pulmonary Stenosis

چکیده

The aim of the present review is to highlight strengths and limitations echocardiographic parameters scores employed predict favorable outcome in complex congenital heart diseases (CHDs) with borderline right ventricle (RV), a focus on pulmonary atresia intact ventricular septum critical stenosis (PAIVS/CPS). A systematic search National Library Medicine using Medical Subject Headings free-text terms including echocardiography, CHD, scores, was performed. refined by adding keywords “PAIVS/CPS”, Ebstein’s anomaly, unbalanced atrioventricular septal defect left dominance. total 22 studies were selected for final analysis; 12 them focused biventricular repair (BVR)/pulmonary blood flow augmentation PAIVS/CPS. All these presented numerical (the limited sample size) methodological (retrospective design, poor definition inclusion/exclusion criteria, variability outcomes, differences adopted surgical interventional strategies). There heterogeneity cut-off values proposed, difficultly establishing which one should be recommended. Easy such as TV/MV (tricuspid/mitral valve) RV/LV (right/left ventricle) ratios proven have good prognostic accuracy; however, data very (only two <40 subjects). In larger studies, RV end-diastolic area higher degree tricuspid regurgitation also accurate predictors successful BVR. These measures, may either operator and/or load/pressure dependent. TV Z-scores been proposed several authors, but old heterogenous nomograms sources employed, thus producing discordant results. summary, we provide currently available risk prediction CHDs diminutive that serve guide use clinical practice.

Pulmonary atresia with intact ventricular septum

OBJECTIVES We describe the morphologic variability in pulmonary atresia with intact ventricular septum (PAIVS) within a population-based study. BACKGROUND An uncommon disease, PAIVS shows considerable morphologic heterogeneity. Clinical reports, based mostly on small samples of patients, may not reflect the true spectrum of pathology of this condition. We have studied the entire range of morpho...

Pulmonary atresia with intact ventricular septum.

It is important to employ a strategy for pulmonary atresia with intact ventricular septum (PAIVS) in collaboration with cardiology and cardiac surgery. We established our policy of treatment based on the right ventricular end diastolic volume. Sinusoidal communication should be carefully considered. Prognosis was poor in case of interruption of coronary arteries. Although catheter intervention ...

Pulmonary Atresia With Intact Ventricular Septum

Background. Neonates with pulmonary atresia and intact ventricular septum (PA-IVS) are frequently born with hypoplastic right heart structures that must grow after right ventricular decompression (RVD) procedures for a complete two-ventricle physiology to be achieved. Previous authors have asserted that neonatal right heart size or morphology will predict right heart growth potential. Since 198...

Pulmonary Valve Atresia with Intact Ventricular Septum.

A case of pulmonary valve atresia with intact ventricular septum' with long survival in the absence of a patent ductus arteriosus, is presented. The patient, alive at 21 years of age, has the anterior descending branch of the left coronary artery originating from the pulmonary artery. Thus, the pulmonary blood supply is obtained from the right coronary artery via intercoronary anastomoses. No s...

[Ductus arteriosus patency with stenting in critical pulmonary stenosis and pulmonary atresia with intact interventricular septum].

We report our experience with stenting of the ductus arteriosus in three neonates. Two patients had pulmonary atresia with an intact interventricular septum and one had critical pulmonary stenosis. Radiofrequency ablation was used to open the atretic pulmonary valve in two patients. In all three patients implanting a stent avoided the need for surgical creation of a Blalock-Taussig shunt. In tw...
